Intake & Outreach
Hui Malama provides intake and outreach services to clients in need of special or individualized assistance. Our Outreach Case Managers provide services to aid clients in the management of their health & social needs. Outreach Case Managers may provide assistance in clients homes, or at a Hui Malama Ola Na Oiwi facility.

- Home visits & Health Assessments
- Community & Health Resource Referrals
- Medical Related Transportation
- Cancer Navigation & Education
- Providing Linkages to Health Care
- Health Education & Classes
- Mai ka Mala`ai ~ Diabetes Education
- Ho`ola Hou ~ Tobacco Cessation
- Malama I Ka Pu`uwai ~ Hypertension Education
- Hele Mai`Ai ~ Nutrition Education
Individual Services include:
- Blood pressure screening
- Glucose screening
- Assist with forms and applications. IE; DHHS, SSI, etc.
- Schedule appointments and screenings for clients
- Follow ups with Primary Care Physicians
